Living with a chronic condition means regular medical care, multiple specialists, and ongoing medication management. In Australia, your GP is the central coordinator of all this care.
Under Medicare, you need a GP referral to see specialists like cardiologists, endocrinologists, or rheumatologists — and to access rebates. Your GP also creates Care Plans (GPCCM/EPC) that unlock bulk-billed allied health sessions.
Your GP knows your complete medical history, can spot dangerous drug interactions, and ensures all your specialists are working together towards your health goals.

What is a GP Care Plan?
A GP Care Plan (also known as GPCCM or EPC) is a document created by your GP that outlines your health goals and the treatment needed to manage your chronic condition.
With a care plan, you can access up to 5 bulk-billed allied health sessions per calendar year, including physiotherapy, dietetics, psychology, and more.
Eligibility: You may qualify for a care plan if you have a chronic condition that has lasted, or is expected to last, 6 months or more.
