Introduction

These Terms of Service govern your use of medical services provided by Reliance Care Medical Centre. By attending our clinic and using our services, you agree to these terms.

Appointments

Booking and Attendance

• Appointments can be made by phone, online, or in person
• Please arrive 10 minutes early for your first appointment
• Bring your Medicare card and any relevant medical documents
• If you are a new patient, please complete our registration form

Cancellations

We require at least 24 hours notice for appointment cancellations. Late cancellations or missed appointments may incur a fee.

Fees and Billing

Bulk Billing

We offer bulk billing for eligible patients. Please check with reception to confirm your eligibility.

Private Fees

For non-bulk billed consultations, payment is due at the time of service. We accept cash, EFTPOS, and credit cards.

NDIS Services

We are NDIS registered providers. Please bring your NDIS plan details to your appointment.

Medical Records

We maintain comprehensive medical records in accordance with Australian health privacy legislation. You have the right to:

• Access your medical records
• Request corrections to your records
• Request a copy of your records (fees may apply)
• Request transfer of records to another provider

Patient Responsibilities

As a patient, you are responsible for:

• Providing accurate and complete medical information
• Following treatment plans and medical advice
• Informing us of any changes to your health or medications
• Treating staff and other patients with respect
• Paying fees on time

Limitation of Liability

While we strive to provide the highest quality medical care, we cannot guarantee specific outcomes. Our liability is limited to the extent permitted by Australian law.
