Managing Multiple Conditions

Many older adults live with multiple health conditions. Managing several conditions and medications can be complex. Our GPs take a holistic approach, considering how your conditions interact and prioritising what matters most to you.

We work with you to create a management plan that balances optimal treatment with quality of life, minimises medication burden, and addresses your personal goals and preferences.

For complex needs, we can coordinate with geriatricians, specialists, and allied health professionals to ensure comprehensive care.

Supporting Carers & Family

We understand that caring for an older family member can be challenging. We're here to support the whole family.

Carer Appointments

Carers are welcome to attend appointments to help communicate concerns and understand care plans.

Clear Communication

We provide clear explanations and written instructions to help with managing medications and appointments.

Service Connections

We can help connect you with My Aged Care, home care packages, and community support services.

Advance Care Planning

Advance care planning involves thinking about and documenting your preferences for future medical care, particularly if you become unable to make decisions yourself. This might include:

  • Your values and what's important to you
  • Who you want to make decisions on your behalf
  • Your preferences for medical treatments
  • End-of-life care preferences

Our GPs can guide you through this process and help you document your wishes in a way that ensures they are respected.
