What is Chronic Disease Management?
Chronic diseases are long-term conditions that require ongoing management. Unlike acute illnesses that resolve quickly, chronic conditions like diabetes, heart disease, and asthma need regular monitoring and treatment adjustments.
At Reliance Care, our GPs work with you to create a comprehensive care plan that addresses all aspects of your condition. We coordinate with specialists, allied health providers, and other healthcare professionals to deliver high-quality, coordinated care.

Regular Monitoring
Regular check-ups allow us to track your condition, adjust medications, and catch complications early. Most chronic conditions require reviews every 3-6 months.

GP Management Plans (GPMP)
If you have a chronic condition, you may be eligible for a GP Management Plan. This is a structured document that outlines your condition, treatment goals, and the actions needed to manage your health.
A Team Care Arrangement (TCA) can be added, which allows you to access up to 5 Medicare-subsidised allied health visits per calendar year. This might include physiotherapy, dietetics, psychology, podiatry, and more.
Allied Health Access
With a GP Care Plan, you can access Medicare-subsidised sessions with:
